When you arrive at Strathroy Family Dental for your first visit, you will be greeted by one of our friendly team members. We ask that you please arrive approximately 15 minutes prior to your scheduled appointment to allow yourself to fill out the necessary new patient forms we require prior to treatment. If you need any assistance or have any questions or concerns while you are filling out your new patient forms, one of our team members will be happy to assist you.

If you would like, you can complete our New Patient Form in advance and bring it with you to your first appointment. Please also bring any necessary information regarding your medical history, dental insurance, and a list of any medications you are taking.
